2. Proof of Present and Permanent Address (Self attested by the parents) (1 Photocopy each and Original for verification) (For All)

3. Transfer Certificate in Original (From a CBSE/ICSE/Govt. Recognized affiliated school). (For Std. II onwards, to be submitted after completion of ongoimg session.)*

    (For outside state candidate TC must be countersigned by D.I. of Schools/Regional Officer of the Board of concerned Region.) 

4. Character Certificate (Self attested by the parents) (1 Photocopy and Original for verification) (For Std. II onwards)*

     *For in-house students (Std. X) of DAV Public School Haldia, TC & CC is not required.

5. Medical Fitness Certificate of the student from a Registered Doctor to be submitted in original (For All).

6. Medical Certificate for child with special needs and handicapped. (Self attested by the parents, if applicable)  (1 Photocopy and Original for verification)

7. Photocopy of SC/ST/OBC Certificate of the Child (Self attested by the parents) (1 Photocopy and Original for verification) (If Applicable)

8. Income Proof Certificate. (Pay Slip/ IT Return/Income Certificate Issued by Local Authority - Self attested by the parents) (1 Photocopy) (For All)

9. Color passport size photograph of the Candidate, Father, Mother or Guardian. (1 each) (For All)

10. Board Registration/ Admit Card (of last session, to be submitted after publication of result) (Self attested by the parents) (1 Photocopy and Original for verification) (For Class XI Only.)

11. Board Marksheet/ Mark Statement (of last session, to be submitted after publication of result) (Self attested by the parents) (1 Photocopy and Original for verification) (For Class XI Only.)

12. Photocopy of the Aadhaar Card of the candidate (For All Classes).

13. Migration certificate in original. (For Class XI Only if migrating from any other board than CBSE)

14. Transfer Certificate issued through BanglarShiksha Portal is mandatory for the residents of West Bengal or PEN number for outer state candidates. (For All Students of Std. I to Std. XII)
